I've been trying to figure out what conditions lead to them skewering me. At first, i thought it was somthing they could only do from stealth. But recently, while palying my Turian Sentinel, I've had them intterupt my heavy melee attack with it (it had snuck into melee range, and I had just finished palying my krogan soldier...).
So what casues them to do it? Is it just random? Cause I'd much rather know what to avoid doing when near one then have to pray to the RNG god everytime one gets near me.
What triggers the Phantom's OHKO attack?

I always figured that it was just being in too close for too long. I typically don't give them the benefit of the doubt.

I think it is related to range, because twice now I've revived while the phantom was over me and it would immediately stab my character in the gut. Very annoying.

I have no idea what causes them to do it and it seems they do it randomly.
This has only happened when I play as my Vanguard and do charge > nova > nova, rinse and repeat. It usually massacres Phantoms with ease but sometimes they'll OHKO me while I'm in the middle of Nova (yes, in the air) while they're still staggered by my Charge. There have been times when they OHKO me IMMEDIATELY after my Charge hits when they should be staggered.
This has only happened when I play as my Vanguard and do charge > nova > nova, rinse and repeat. It usually massacres Phantoms with ease but sometimes they'll OHKO me while I'm in the middle of Nova (yes, in the air) while they're still staggered by my Charge. There have been times when they OHKO me IMMEDIATELY after my Charge hits when they should be staggered.

Yes I think it has to do with how much time you spend near them. At first I though it was only when you had no shields, but that isn't the case, as I got killed with shields. I guess there is a timer, and when you stand near them too long, they swap their regular sword attack for the OHKO.

I think it is touching. That's why I just charge and immediately begin backing up, firing my shotgun as fast as possible.

I don't think it's time because a couple of times, I have used Charge from medium range then they OHKO me the second I'm close to them. Sometimes they even OHKO me the second my Charge collides with them.
I'm thinking it's just an ability they have and they can do whenever, because I've been pretty close to them for extended periods of time and they don't OHKO, instead they just kinda flip around doing nothing

I think like the other people said, it has to do with range.
I know multiple times I was reviving people, to only be stabbed in the back.

I don't think so, many times they've OHKO'ed me just after I charged them.
