Everyone:
... it's infuriating!
My female paragon Shepard has a paragon score that's almost maxed out, and *still* the dialogue option for a paragon response was grayed out. Subsequently, I lost Jack's loyalty because I sided with Miranda. (In my other playthrough, I sided with Jack and lost Miranda's loyalty.)
What does person have to do in order to enable the paragon and/or the renegade response options? Does paragon really have to be at 100 percent?! If so, what the heck were Bioware software engineers thinking?!
Like I said, it ticks me off that the paragon response is basically impossible for me to get. I'm not playing a third time through ME2; twice is enough, darn it.
Sigh. I was hoping for a picture perfect playthrough for an ME3 import, but both of my ME2 playthroughs have a "scar" now — each one, that damnable Miranda vs. Jack decision.

I sided with Miranda, then went to chat to Jack. There was a paragon choice to make and it re-gained her loyalty. I'd assume there is also one for Miranda.

Im having the exact same problem, Im doing an insanity run through with a imported me2 shep, trying to have a perfect save for ME3, but this happened to me also. I sided with Miranda this time, pissed off Jack and now I can't kiss and make up.
I've done pretty much all the main content so I dont think I could even get my paragorn up any higher.
The stupid thing is, on my normal run through I had no problems at all with Miranda and Jack and had 100% loyalty for every team member, but Kelly died in the collector ship. grrrrrrrrrrrrrrrrrrrrrrrrrrrrrrrrrrrrr

I lost half of the Normandy's crew in my first playthrough of ME2, but kept everyone else alive in the suicide mission (even a non-loyal Miranda).
As for my current run, I'm just going to finish the remaining content and try to make it up to Jack right before the Reaper IFF mission. If it doesn't work, then so be it. Still, it's stupid how high the bar is set for this particular paragon/renegade check.

Its not the height of the bar that makes the choice go through or not, its the ratio of paragon to renegade.
Basically, this is put into the game so that you play ME2 the way that the developer wants you to. They control what mission when more tightly than they let on.

it's number of paragon points compared to the number you might possibly have...I do not know if 100% is enough or if you have to maximise your "class skill" which gives a big bonus to persuasions.
It's much easier for renegades (the Legion/Tali dispute is the other way round)
In my first playthrough I picked Jack, and straight afterwards went back into Miranda's room and charmed her into thinking I was lying to keep the crazy girl in line.
(2nd playthrough was renegade so intimidated both, 3rd was mostly paragon with maxed class skill so both charm and intimidate options were available)

Yea, it's not the total amount of paragon points you have, it's based on a percentage paragon points you have in relation to how many you could have had if you picked all the paragon options up to that point. I think... 
The best way I've heard of getting it is to go completely paragon (or renegade) through Horizon, then do Jack's loyalty mission as soon as you get it (that should be right after Horizon). Also, do not visit other places like the Citadel until you need to go there. I'm not clear on it, but I think that adds to the pool of points from the missions there for the paragon checks.
As an aside, I found it very annoying to lose Miranda's loyalty, especially after saving Oriana. I never could get the 'make up' paragon check to light up, even after doing everything before the suicide mission and LotSB to boot.
