If I could choose from all of them, I'd choose Miranda and Jack, because they're my two favorite characters ever, but from the ME3 squad only, I'm not sure yet, but I think Kaidan, Garrus and Liara are the most useful.

I can't say yet, I usually pick my team based on what I expect to be useful for the mission and what characters are most useful. It made me sad that I barely used Tali in ME2.
If I had to guess, I'd say Garrus and Liara, but we'll see.

If I can take Legion, it would probably be Legion and Prothy all the way.

It'd be awesome if, at least, for the final stretch Me3 made every squadmate from Me1-2 available. That would be amazing and I'd bring Kaidan (best friend/rival) and Miranda (love interest) for my Shep hands down.
As it is with the Me3 squadmates, it'll likely be EDI/Garrus. Have an alternate infiltrator Shep where I'll be running a James/Kaidan brogroup as well.

Ashley and Garrus. They are my team and I want them with me at the end. One for brofisting with at the end of the battle and the other I intend to make out with endlessly in a broom cupboard at the award ceremony.

It really depends on the plot progression. If I feel like certain squadmates have gotten a lot of development (or pseudo-development via headcanon, or... something) I'll bring along the underused or the most likely to volunteer for the big finish or something along those lines.
If all else fails, I'm most likely to bring Garrus and either of the 'spoiler' squadmates by default, I'd say. I tend to think of Kaidan as capable of keeping things together for the rest of the team, Vega I think my Shepard would want to sort of protect from his own potentially self-destructive tendencies during a big suicide-esque run, Tali I tend to view as more useful in engineering, and Liara... I don't know, probably the most likely to take one of those spots, but handy enough sporting intel from the sidelines.
