Occassionally getting disoriented when I get downed so that when a teammate rezzes me or I use medigel, I lose all sense of direction and wind up running around like a chicken with her head cut off until I can re-establish exactly where I am (embarrassing b/c in RL I have an ironclad sense of direction, but it screws with me in games). Hyjinks ensue.

doing the vanguard dance with a turret only to realize that the bastard engineer finished setting another one just behind me...

I would say the stupidest thing I've done was not specing up Nova higher and sooner, I'm just discovering the power of Charge+Nova and it's glorious...although it could've been more glorious.

Revive someone in front of a turret.
Miss a melee attack and got stab by phantom.
Got killed in the first wave when played as a krogan.

Run blindly into turret fire to rez a teammate, only to die myself.
It is bizarre how often this happens, it's like people think that just because you are reviving the turret will not notice you...

Oh... and played as my Drell Vanguard for the first time in a Silver game. We beat it, but I had nothing to do with it since most of the time I was horizontal / running away...

Holding Spacebar to run in close quarter and getting in a cover in the crossfire of a turret and some other enemy. Dieing.
Pressing Spacebar to roll away from a grenade and getting in a cover just on top of that grenade. Dieing.
And the list goes on...

Sometimes they don't when locked on something else. But yeah.

I haven't really done anything stupid, but people I played with have.
I was playing my Krogan Soldier, walking around blowing people away with a shotgun, not a care in the world, then suddenly all three of my teammates decided that THEY wanted the kill so they dove out in front of me only to take a shotgun blast in the back. All three went down. I killed the guy I was after then walked away. This is the most conceited thing I have ever read. Just saying. Isn't even true since our weapons don't injure teammates as far as I know.
I don't see how it's conceited. If you're going to charge straight into friendly fire and die, I am NOT going to help you.
Maybe it's a glitch, but on the PC version there are a few different "Friendly fire!" sound triggers and I have lost shields and got sniped a few times by my supposed friends.

Mine.
Stayed outside the extraction area for a better view of enemies, died, a teammate came out to save me, also died, she got immediatly stomped, I got revived and we lost our full extraction bonus. I apologized but needless to say I got kicked, and rightfully so. I stay in the LZ in round 11 unless someone needs reviving now. I learn from my mistakes. Other then that monumetally stupid moment, I roll into grenades, charge into the path of turrets (You really should read what exactly kiled you team member) and wasted ammo and played cat and mouse with a phantom while the rest of my team looked on, damn spectator mode. When nervous I can't aim so I stasised that ninja and proceeded to riddle the ground near her feet with bullets.

LOL! I do the same thing when nervous...I start shooting like i've been roofied

Had to get to an ammo dump to keep on firing (well not really, I have like 20 ammo packs stored away but I almost never use them)

Trying to climb a ladder under fire and instead keep rolling into it and eventually getting downed.

As a vanguard, oh boy... too many to choose from. I'd have to go with the time I tried to charge an isolated trooper. No problem, right? Well, charge failed to acquire the target, so I hit the button again. Except for in between key presses, the trooper got his head asploded by a sniper. I ended up charging some centurion off in the distance, who just happened to be directly between a turret and a phantom. Needless to say, I spent the rest of that wave spectating.
