Glitch? Someone died on the SM, and yet, they didn't die on the SM. wtf?

So yeah I was playin' some ME2, remaking a few of the decisions that didn't feel right for the character of my canonical Shep.  At last I reach the SM.   It all goes just fine.  Only Zaeed and Legion are not loyal (saving Zaeed for later so I can kill 'im, Legion because I hate only one race more than the geth (Batarians) and so I didn't feel like straining the character and saying he lied in taking Tali's side when he totally wasn't.

Holding to the promise I made myself prior to my first SM run (I won't "cook" any of my decisions to get a better outcome, I'll hold to them to get the genuine import for ME3), I begin.  Tali on the door.  Miranda leads B-squad every time.  Samara puts up the anti-seeker ME field.  Garrus and Miranda for the final fight, Jacob takes the crew survivors back to the Normandy, and everybody else holds the line.

This is the exact same as my last run, and my last run had zero casualties.  Whenever it is that Shepard tells the team holding the line to fall back to the Normandy, well, that happens.  I later get a report that there were no casualties, as expected.

...then after I tell TIM to go suck a bag of dicks, I see Shepard standing over a coffin.  Whaaa?

Next time I go out, Legion can't be picked for the shore party.  Because he/it's dead, apparently.  Even though I didn't see him/it die, and was specifically told that I had no casualties.  Not to mention the fact that he didn't die on my identical first run of the SM, and I'm 99% sure I didn't patch up with him after the argument with Tali that time either.

What the hell, man?
