Examples:

HACKING
good: at least two guys are in the zone at all times
bad: one guy is pinned down while the others run amuck

BEGINNINGS OF WAVES
good: everyone gets into position and is vigilantly covering probable enterances, before spreading out to decimate the enemy
bad: everyone runs around as if this was an every-man-for-himself FPS, and getting themselves killed because no one can back them up

COMBAT ENGINEERS
good: if you see him you take that bastard out, it is a priority
bad: engineers are unchallenged and setup turrets everywhere

Good: Take cover whenever possible
Bad: Stand out in the open; get chopped to pieces and/or stand in my scope

PHANTOM
Good: Coordinating with your teammates so only two of you try to take it down
Bad: On first sight of phantom everyone rushes to kill it and nobody protects your back

Good: stick reasonably close to your team mates unless you are class designed to go solo causing trouble. This means we can back you up, revive remove shields and barriers, active biotic combos and so on. Remember this is a coop game. The powers and classes are designed to work together.
Bad: run off on your own meaning the other team members have to go on mad dashes to revive you or help you with an atlas, turret or other tough enemy.

I'm the sort of player that wants to support, I'm as happy with assist and revive medals as headshot/kills. Give me the best possible chance to help you. :)

If you break line of sight with your team, you're doing it wrong. (I'm looking at you, fellow Vanguards)

bad playing: use self-rez when killed
good playing: use self-rez only in bad situations(ie. most of team is kiled or the're really lot of enemies), otherwise teammate may rez you

bad: use rocket laucher as soon as see atlas
good: use rocket launcher when you're overrun

Don't hug cover like it's the last thing in the world. Put walls/objects between you and the enemy to break LOS but stay mobile.

Always get back at extraction. 4 against the waves are better than three and one troll halfway across the map. Sorry, you're not getting revived when your character dies.

Depends on the situation actually.

@Topic:

Good: If there are objectives to handle, the whole teams instantly goes for them.
Bad: You're delaying yourself by killing of enemies that'll respawn moments later anyway.

Good: If you see two dead teammates close to each other, be extra careful. Something killed them - most likely a turret or Atlas.
Bad: Blindly run over to rez and get killed as well. Actually this part is valid for basically any rez.^^

Good: If you hear a turret, take it out asap.
Bad: Forget about the turret and get killed by it a minute later.

Good:
Do not bunker down at the LZ immediately in the last wave, you'll just get swarmed. Hold the more easily defendable positions near them and fallback to the LZ in the last twenty seconds, has always worked!

Good: Turn your mic on and communicate!
Good: Take a note of the classes of the other players, and try to think where you fit into the team.
Bad: Stay silent whilst watching a Phantom run towards the back of a player you can't get to. Or, actually, stay silent in general
Bad: Trying to kill absolutely everything to be top of the scoreboard, even if that particular enemy is already helpless and there are another 2 who are actively hurting your team.

From my last game (which I just ragequit):

AWARENESS
good: Taking a moment in between shots to check your not being flanked.
bad: Having a single Centurion take out the other 3 members of your team singlehandly WITHOUT BEING SHOT AT ONCE (saw it walk out of the building with full shields).

Definately.

I would add though, if you are playing bronze you have theoretically 8 rockets and never encounter more than 2 possibly 3 atlas. Here you might as well use them, almost on sight, but if you are going to and have mics on tell the team you are going to use a rocket, means you don't end up wasting multiple rockets on one target.

On silver/gold, different ball game. You need to ration and use all 8 rockets effectively. This means communication and not using a rocket when you could happilly take it down using conventional means.

Good: When in a silver/gold lobby analyze what the other classes are. If there are two people of the same class don't be that same class as well. The only time where two people are the same class is acceptable is if they are soldiers (1 krogan and 1 human) or two adepts (increased rate of biotic combos and CC).
Bad: Everyone is the same class and thinks they can destroy anything and then wipe when that one enemy they are all weak against shows up and decimates them.

In short, variety is rewarded while similarity is not!

ROCKETS:
Good: Talk to your Teammates before you shoot a rocket in any case
Bad: All players shoot their rockets at the same target. GG -.-

Good: Always stick to your teammates
Bad: Wander off your own and get killed, team tries to revive you, team fails

Resurrecting

Good: Resurrect a teammate when it's safe to do so.
Bad: Running in without noticing the swarm of enemies around and dying before barely making it a quarter through the resurrection.

Good: Keeping track of what your teammates are doing and taking out enemies that they're not focused on.

Bad: Needing to get EVERY. SINGLE. KILL. So much that you rob kills (and therefore xp) from your team and allow the group to get flanked.
