 I have no doubts that many characters will die in mass effect 3, your choice or not. I Also have no doubts that bioware will make them frigging amazing !!! 

Can you think of any scenarios ?

E.g i think Vega's Reckless-Soldier persona will land him in a whole load of trouble.

And what do you think about a Armageddon like sacrifice from a squadmate at the end of the game

I'm pretty sure only 2 or less people will have unavoidable deaths. Then everyone else will have avoidable deaths.
