Mine are:

Weeping Angels (Doctor Who)

When not being observed they move very fast and turn to stone when somebody sees them. They send their victims to the past and feed off the energy of the years the victims would have lived in the present. Blink and you're dead.

Guardian (Dead Space)

Creatures who are fused to walls and guard doors. They cannot move and spit out embryo-like projectiles that attack once you approach. It also has a long tentacle formed the intestines, which kills you immediately if you move too close. They are quite disgusting in nature as they are somewhat still alive and not dead like other necromorph and constantly live in agony and pain.

I always thought Pumpkinhead was one of the creepiest monsters.

I have no idea what those creatures in "Amnesia" are, but in Finland they're called "Morso"

Silent Hill 4 The Room: The ghost in the shopping mall world i think. When he attacks you he stops then rewinds like a tv show. Or any ghost from Silent Hill 4 is scary.

The Grudge game for the Wii, found that ghost woman in some small room, starts crawling towards you slowly and then speeds up.

Pretty much any Asian male or female that becomes are ghost are scary.

I remember we had this on VHS when I was a child, but didn't get around to watching it until earlier this year. After seeing the birth scene, I dodged a major psychological bullet.

Kentarou Miura's monsters/Apostles from Berserk. Guy really knows how to draw nightmare fuel.

Also the alien from the movie The Thing(1982) was pretty creepy with its shape-shifting. Not knowing who you could trust.

It's a David Cronenberg film, so twisted mutations and gory special effects are to be expected. The Fly was pretty grim too.

 I haven't ever been creeped out or scared by anything really, but I do admire the Xenomorphs they are a much better species than that of ours.

Pennywise the clown from IT still creeps me the hell out. But yes, Miley Cyrus is worse.

And also animated clothes with no people in them. There's a Dairy Milk advert that has clothes dancing around a charity shop. Not good for my nerves.

That's not Pyramid Head. That's an imposter. The only real Pyramid Head is the one in "Silent Hill 2"

This right here.
I don't care that its easy to exploit them.
As soon as I start up the game I forget about throwing chairs at them and I'm panicking like nobody's business.
The ones later in the game are even worse.

My answer may seem like a cop out...

People. No, really. The moments that creeped me out most in films were when a person committed, or hoped to commit a truly deplorable act. In some cases--particularly when the story was based on real world events, I felt disturbed enough to feel that much less trusting of my peers.

I have forgotten gross out monsters days after watching a movie. They lack the staying power to truly creep me out.

The only monsters that creep me out on a regular basis are remarkably enough, zombies. Always hated zombies, but can't bring myself to avoid zombie stories. That said, I usually get over zombie stuff by the next morning. It doesn't really count. Most monsters are straight up disgusting to me; not creepy.

I have only seen bits and pieces of Irreversible. I could never bring myself to watch the whole thing. There is media I refuse to consume to avoid permanent emotional scarring. I doubt any fictional monster could creep me out as much as say, anything in those Unit 731 inspired films. Which I also chose not to watch long ago.
