Because when you meet Flemeth in DA2 it's just when the blight destroys Lothering and not long after your warden in DA:O left her swamp with Morrigan, which is rather early on in the game. [if you recall when Wesley falls sick, Flemeth says the only way to cure him is to become a Grey Warden, and Hawke can say "and they all died at Ostagar" to which Flemeth says "not all, but the last are now beyond your reach"]
So at that point in time your DA:O character hadn't been given the option whether to kill Flemeth.

With regard to her appearance in Act 1, once Hawke completes the task Flemeth requested, the task itself explains how and why she is there. "...a bit of security, should the inevitable occur."

2) As explained in Dragon Age: Origin, Flemeth has figured out how to extend her life. Hawke apparently helped this time.
A better question is: If the Warden didn't kill Flemeth, is there more than one flying around now?

I think about this same thing regularly. I wonder what happens if she comes across herself?

I have a feeling flemeth is killed by morrigan with or without the wardens help eventually. its all part of her plan

Morrigan explains that Flemeth is still a problem after you kill her.
Serouisly though, that was so frustrating, having to rescue flemeth after I killed her in Origins.
