If that rumored Single Player DLC is true. I'm betting we'll see another 'mission' like in ME2 where you play as Joker.
Only we'll play as either Hackett, some squadmate, or EDI.
I'm thinking the cutscenes we see are real and that the DLC would expand them.So Shep dies and EDI's like "i can interface with the Crucible and fire it." That then explains why the Normandy left. The cutscene showing it could be them leaving for it.

In all most every show where someone dies they show the person taking one last QUICK INHALE. So in the 'Red Choice' Shep could have taken one last breath.
Furthermore, we don't know what all that Proth Beacon did to him back in ME1, it may have activated some latent Psy Ability, the Proths do mention that Humans are pretty advanced. And many species comment on how Humans have come so far in such a short time.
So, back to 'Red Choice'. Shep could have willed the 'Red, Blue, or Green'. And the fact we see him is just his mind "making sense of it". It could be that in Blue and Green he's consciousness is merged with the Reapers either as a co-existence (Green) or a domination (blue). In the 'Red' he may have willed something to go wrong.
And so the 'Red Cutscene' showing him under the rubble is actually him "letting go".
**Now the Normandy could be either returning to the fight after activating the Crucible, which we still don't have much of an idea of what it does. Or they could be on their way there.
Basically, in the end, I don't think there's going to be an option for Shep to live
The "breath" scene definitely looked like a resilient inhale to me. Not a sigh of relief. It'll be interesting to see what it is "supposed" to be.
