Actually it still is!Completely ridiculous this went unnoticed!!
That bug has been around forever. ME1 imports generate wrong facecodes. DO NOT USE THEM.
The only ways to get proper facecodes for ME1 imports are listed in the OP.

I planned out playing ME3 today. Now I won't be doing it because the one character I want to carry over won't look nearly the same.
So I'm boned. I've got to find other things to do. Besides the usual eat, sleep, clean, etc etc, I'm gonna be bored.

ME2 generated faces do import because they have a code.
NONE of the ME2 face imports of ME1 characters work because the code was lost???

This was confirmed, right? So this means my precious canon femShep is lost.....
I am very very mad right now!
Unless they patch it, the thing I was looking forward to the most got ruined.
It may not be a big deal to some people, but the face is crucial to me. Sure, what Shep says and does will be the same. And your choices do get imported. Fine. But it FEELS different. It was such a great moment to see my Shepard's face in ME2. Like seeing an old friend again.
I feel emotionally attached to my Shepard.
This is the same thing as having an actor in a TV show replaced. Would you keep watching? It is almost never done because it would mean the end of the show. They prefer introducing new characters rather than pretend the main character just had a "Face/Off" like plastic surgery.
Well, despite my anger it looks like the problem dates back to ME2 development. They missed to preserve the ME1 face code.
So it's actually not the fault of the ME3 testing team or laziness. I'll cut BW some slack.
I work in Q&A for a major game developer myself btw. And let me tell you that the level of influence testers have is rather limited. Not everything that gets spotted and should be fixed gets fixed. Some fixes would consume too many resources or deadlines must be met.
Still, it should not have happened in the first place. This is a major issue! Not being able to import your character's looks beats the whole point of a character-driven trilogy!
Fans tend to have expectations far removed from the reality of game development but the import issue is a KEY ELEMENT of the game!
BioWare f*cked up big time.
It's sad. Really sad.

Doesnt matter how long the bug has been around. This shouldnt have happened. Thats why this thread is generating a lot of feeling and has thousands of replies.
As for the fixes in the OP people shouldnt have to resort to that. Additionally some of those are tricky to do for console users.

Its like the main character of a TV show you love being replaced, but without a storyline reason for the change.
So, imagine House M.D. but without Hugh Laurie playing the role of House, and in his place for the final season is...Michael Clarke Duncan.
Doesn't make any goddamned sense, right?
