However, Slam can also be used as to create a biotic explosion, although the explosion occurs when the enemy is slammed and not when it first lifts the suspended target. You can see it used a few times in this video by capn233.

I've been playing MP mostly since the game came out, so I've kinda forgoten about both Slam and Dark Channel. I will add them both, since all the other things listed in the OP apply to single player as well.
EDIT: Done.

I meant the specific power used to set up the combo. E.G. Overload --> Overload will not cause a Tech Burst, but Overload --> Energy Drain will cause a Tech Burst.
Tech Burst and Fire Explosion work on all types of enemies. Cryo Blast doesn't work on armored targets, since you can't freeze them.

Actually Sabatoge does do damage on non synthetics(back fire), so it might be used to trigger some of the other combos. Haven't tested myself.
Also with Energy drain it only tech bursts if the shock happens so a good deal of the problems people are seeing with it is that its not shocking. Overload has a higher stun chance and neural shock basically garuntees that you can do tech burst. Though the window is still smaller than the effect of neural shock.
Interesting note, Cryo -> Incinerate kill triggers both a cryo explosion and a fire explosion.

Submission Net can set up Tech Bursts. Biotic Sphere can set up and detonate biotic explosions. You can find more info in the OP.

No. The only thing warp ammo does is profit from additional damage when the target is under the effect of any of the priming biotics. Be happy. You can set a primer, riddle them with warp ammo while waiting for the cooldown and then detonate.

He means Warp>overload>throw>throw leads to a tech explosion and then a biotic explosion. The overload does not prevent the biotic explosion, just delays it by layering a tech ontop of it.
Warp>overload>throw>Energy Drain>throw>overload>throw>throw leads to 3 tech bursts and a biotic explosion. Done that a lot (warp lasts a while, and 2 people with tech setup abilities can delay the biotic boom a lot, but with every throw adding zappa I don't mind one bit).

Incinerate does not block biotic explosions for non-fatal triggers (for fatal triggers, I do not know).
One time I was going with my usual warp+throw combo, but there was 3 engineers on my team (who in their usuall ****yness would all concentrate their electric attacks on the geth primes with armour, instead of the other guys... thanks..). There was simply no way that I could throw through 3x energy attacks, however, some of the time, one of the engineers used incinetrate which would create a small time window which I could detonate the biotic explosions.
(The incinerate would set off any existing tech burst, and it also demonstrates that it is non-blocking, for biotic explosions)
