This is a failure on the part of the team that programmed the import process.
They should have looked into, fixed, and adapted, so that the ME 1 appearance would translate to an ME 3 face code. Instead, they chose the lazy way, failed to address the issue, and require those of us who *have* carried a character through all three games, to reconstruct the issue.
Does anyone have a way to discover the settings that replicate an original ME 1 face in ME 3?
Appearances carried over from ME 1 do not import

There is a fix for this.
Download the patch. I repeat, download the patch. You will have to do so manually; unlike Steam, Origin neither tells you there's a patch available nor pushes the patch to you.
To download the patch *right* click on the ME 3 icon in Origin or in your Game menu and select "Check for updates."

again, I don't understand this...this is the FINAL game of this trilogy, and yet, full of problems BEFORE(all those thing about endings and gameplay thing in general like less wheel conversations...etc) and now AFTER, fail imports...*sigh*
And not to mention after I updated the game, I got release date check again, I thought that thing is removed after the first check?!

The day-one patch is in the preload if you did it that way. Still hasn't fixed my Shepard. I'm really bummed...but at least I went multiplayer and boosted war readiness a little.

there is a workaround somewhere in the genral forums for the faces from me1, it works pretty good, but you have to invest 15 min or so...

The "Day One" patch is *not* in the preload. I had to do the following on Windows 7:
Open up the Games folder. Right click on Mass Effect 3. Select "Check for Updates."
The other method is to open up Origin. Right click on Mass Effect 3. Select "Check for Updates."
The other thing is, the patch will *not* correct character appearances for characters that have already started the game. You have to import from scratch after making sure ME 3 is up to date.

On further testing:
The patched game only properly imported *one* of the four FemSheps I had carried over from Mass Effect 1. The others are still "Please reconstruct appearance."
This is really, really, REALLY bad, Bioware. You could have, should have, done better.

Updated and it solved nothing. 
Problem is the character creator doesn't even work like it did in ME1. At least I noticed a big difference in how nose depth works now.
Oh, and there are options missing.
