It seems I missed the sixth vial, the one you can find during a random encounter in Denerim. I have only the last quest left now and I can't get any more random encounters in Denerom. Is there any other way to get to it? Otherwise it's lost for me.

I started over because of that.

In the end it was pretty pointless.  You get no story, no quest, no rewards from getting all 6 vials.

Actually you get a shield.

The Shield is from the Unbound Quest. That is not the vial quest. At least that is where I got the shield from XD

I missed it as well, the problem is I got the kill D or K or whoever quest and now everytime the vial area should fire I get that area complete with corpses. It was a little annoying must have got it 15 times as I moved about.

Actually, that Revanent (the one in the Denerim encounter) DOES drop a shield, the Dead Coat of Arms. It's a pretty crappy shield, however, so the OP isn't missing much.

 I just made a quick mod that will take you back to the revenant area: http://www.dragonage...ile.php?id=3007.  I only tested it on my machine so if it works for you too (or not) please post some feedback.

Yes, you just miss out on a codex entry and a shield you would sell anyway. Plus a small amount of XP.
