Yes, you need Gibbed's editor.  You can also add deafult femshep's face textures which I think looks nice too.  My Shep on the previous page has both hair and face textures modded and I'm on xbox.

I'm wary about posting any kind of modding instructions - even cosmetic - since this game has multiplayer.  If you check on Mass Effect Faces you'll see instructions in the comments sections.  You can also import a headmorph on xbox so if you see a face you like you might be able to just import the whole face.

Just got the game recently but after looking around on the forums decided to add my original Shepard as well (was originally planning on going with default as have every time atleast for first time around but might as well try something new).  Both with and without his Soul Patch (was originally just playing around with editor wanting to create some sort of familiar face but liked the look--just his patch at times is too distracting even if it does add at others).

He's very Default-heavy influenced with a heavier/darker/older look of a soldier that will do just about anything to win.  Playing him as a Vanguard (Default will be adept--as I always make Default Adept first).  Will have to create a female Shepard later.
