Been following this thread since the mid 50's and just downloaded the patch this morning and so far, no change to my screwed up Shepard. Both the male and female Shepard look as they did pre-patch but not in previous games.For what they look like (via my tumblr):

To say I'm disappointed with all of this is quite the understatement. I remember back when I flipped on ME2 that I was worried that my Shepard's look wouldn't transfer over but after it did, I had found that I should have had confidence in Bioware not to screw up something like that since it means quite a bit to the player's experience. Which probably makes it harder to to take this since I had that confidence in Bioware but found it a bit shaken after my Shepard's look didn't transfer and a patch which stated would fix the problem did nothing of the sort.
So I'm still debating on what to do but I'll probably just try and recreate my Shepard as best as I can and let Bioware try and build that faith in them back up again.
It looks like your face is just defaulting to a preset, not even trying to copy. That's what one of my femsheps did: she defaulted to a face that looked nothing like her. I came on this thread and found another person who got the EXACT same face, which is how I found out it was a preset. Other sheps I imported at least "tried" to match up though; I don't know what defines whether it does or not.
This seems to be the result when you imported your ME1 Shep into ME2 and briefly went to the customization screen. Even without changing anything, that step seems to attribute a preset face code to your custom Shepard in your ME2 saves. Since the game (ME2) uses the headmorphs after the imports, the face code is present in the save file, but without consequences to the game.
The ME3 importer looks at the files, finds the (wrong) code and uses that. Only values that are no longer available are approximated according to headmorph data.
You can check if that's the case for you by openign your ME2 squad screen and see if it displays an "identity code" for your Shepard (upper left corner).
This particular problem seems to have been solved for many PC players by using Gibbed Save Editor to delete the (wrong) face code from the ME2 save before import into ME3. The importer then looks at the (correct) headmorph data and approximates from those values. In most cases this resulted at least in a close-but-not-quite or even recognizable Shepard.
The Gibbed method can be used for xbox, too, there is a How-To
here. However, I have no console and thus never used this method. I would like to ask anyone who is familiar with this method to be so kind and share their experience.
