I wanted to clarify something:
Those of you that imported your character from ME1 to ME2, if you just accepted the imported face As Is, then yes you don't have a face code for that character and ME3 can't read it. However, if you just selected Customize and THEN accepted the face you imported, a face code would have been generated.
It's not Bioware's fault and I understand there's no way one could have foreseen this, but it is what it is. Go load ME2 and ME3 side by side, load your ME2 save and then use the face as a reference for ME3 if you really really want to get the same look you had through the first two games. It's totally possible.
I had to revamp my FemShep in ME2 anyway because the complexion was all wrong and I didn't much care for my results in ME1 in the first place.
It is Bioware's fault and of course they would have foreseen this. How do you think the ME2 importer works? It pulls the raw face data from your ME1 end save and just uses that. You know, all the facial geometry and colour vector information etc.. With ME3 it looks like they went the lazy route and decided to just look for a facecode - that was the absolute wrong way to do it.
The other issue here is that plugging in an ME2 facecode doesn't really work anyway (even with tweaks) because they
replaced a lot of features. A lot of people can't recreate their Sheps - it's not always possible to do (perhaps you got lucky).
My Shep cannot be recreated mainly because they replaced the
default female eye shape with something very rigid and unsmooth, they replaced the eye makeup with almost none at all, and the iris colours have (yet again) been replaced. This is unnacceptable when characters like Jack, Joker, Hackett, Ash, Kaidan, Miranda, etc. can all come into ME3 with the same faces as before.
