This is a copy of a conversation I had with Origin's Live Chat.
Items may be bought in game with Origin points (which have a real currency cost).
They may also be purchased with credits earned by playing.
I purchased a multiplayer "Spectre" pack and received a message that there had been an error.
The item was not given to me but the points were deducted from my account.
I was instructed to contact customer service if the re-occured.
This error had happend several times (during the Demo version).
Since this was an expensive error in terms of the game time required to earn it, I contacted customer support.
You are now ready to chat with Prateek.Prateek:
Thanks for contacting EA Help! My name is Prateek how may I help you?
you: I played the demo extensively.
Prateek: Okay.
you: There was an issue in the demo and it just happend in the retail version as well.
I lost 60000 multiplay points in the store on a spectre pack.
Prateek: May I know the game please?
you: Mass Effect 3
Prateek: I would request you to post your issue on the me3 official forums for further assistance.
you: So there's nothing you can do? It makes me very hesitant to pay money for store points if this will continue.
It took me 3 hours of multiplayer to save that many credits.
I sure won't spend real dollars in the store if the same can happen.
This was a common issue in the Demo.
EA should have some process in place to handle it.
Prateek: At the moment we do not have any possible solution for this issue, if the issue gains the developers attention then I can assure you that the issue would be resolved.
you: Please file a service request logging this.
Prateek: And the way to make the developers look in the issue is to post on the forums.
you: I work as a programmer and this kind of thing happens I know.
Prateek: I will definitely file the service request.
you: But that won't get my 60000 points back, nor will it allow me to place any faith in the Origin store.
I will not give my Visa to Origin for in game purchases if there is no mechanism for handling errors.
A forum is NOT the place to handle issues with money.
Prateek: As the game is new, I think you can understand that bugs arise with new programs, and the patches are released for fixing the bug.
you: I do understand there will be issues.
And this is where we should take care of them.
Handling bugs in a forum is fine.
Money and credits need to be handled with more customer care.
Prateek: You would definitely get the money back if you have lost it after the bug is fixed.
you: But I won't get the credits back for an intermittent issue that happens before a patch?
Prateek: You would get the required item.
you: I'll post this conversation on the forum.

Prateek: I would suggest you to make a new thread for the issue.

you: Fyi, I bought the Ashes expansion. The address gave an error where it would not recognize my city.
It said that the purchase failed, but the expansion was later added to my account.

This exact same thing happened to me. It happened yesterday so the problem still persists.

I think he/she/it meant 'you'll get the money back after its fixed, if you lost it' and not 'if you lost it after the bug was fixed'.
That said, it could be true that she thinks you're referring to a different bug, where unlocked items are inaccessible, and that those who suffered from your particular bug and also did not use bioware points might not be getting squat for reimbursement (I'm not rooting against you, I just hope they can actually determine what items you ought to have but don't). All things considered it looks like we're all just sitting here hoping it doesn't take yet another two weeks for some sort of conclusion to be reached.
