My femShep saved her as well though I'm not really sure how it happened.

Just make sure you go to all the meets. Shep mentions Kai Leng in random chat and that warns her. There's nothing that you specifically click to warn her.

was just part of the dialogue tree really.
I haven't got a save with an unloyal miranda so the only way she can die is if you make a bad choice move with her father and oriana and don't make a renegade interrupt I think.

just need to make sure u go to every meet and talk to her when the opportunity arrises, but also give her everything she asks for. eg: access to alliance database.

she lives if you visit her all 3 times she mails you to come to the citadel. On the third time kl is mentioned and she takes some extra precautions. Just wish i could save mordin and thane 8(
