With speakers, why does the games noise sound so muted and quiet yet explosions blow my speakers up?
And for me the biggest issue is when I play using my headphones- the game is intolerably quiet.
And yes, I've checked all my computer sound settings, everything is cranked up to the max. Is there some file I can edit to boost the game volume value?

I suggest you update your audio drivers if you are having problems with the game or you can try and force stereo only playback.

Change the dynamic range setting to Low. "High" only works well with higher end audio equipment.
Changing it to low compresses everything to a similar volume. High has full dynamic range (meaning dialogue is much quieter than an explosion as an example, like real life..)

The only thing that sounds abnoxiously loud for me is when warping between systems. Everything else is well balanced, when I want to warp to another system I usually turn down the volume on my system first.

That's exactly what I have as well. A video of going through relay is too loud. And most of the other prerendered videos have very high volume compared to the rest of the game. I'm pretty sure it's a bug and needs fixing.

I have this problem as well. forcing stereo would be good cause every dialog seems to come trough the center speaker yet I almost hear no background noise and explosions are mainly coming troug the subwoofer and front speakers. But there's no option to set it into stereo from what I can tell. Is it a commandline option maybe ?

I just hate how gunfire actually silences the music for a short period of time, it's very frustrating.

Low-freq distortion/buzzing on some sounds?
Some explosions ridiculously loud?
Joker impossible to hear sometimes?
This is the cause.
Sadly, turning the Audio Dynamic Range to Low also causes the loud sounds to slightly mute the other sounds.

If you got a decent audio setup (good sound card, recent drivers, good speakers) the high setting is no problem.
Look into what kind of sound card you are using (onboard, which usually sucks or a dedicated sound card).
If possible get a dedicated one for PCI-E (not the older PCI bus) should your mainboard support it and have one free slot and if not get the newest driver you can get anyway, often it's possible to mod drivers, say your card/ chip manufacture is not offering anything newer for an older model but for a similar or in some cases even essential the same hardware (only rebranded), often these can be used with some tricks.
Also make sure you turn off any fancy software audio features, like some "clarity enhancements", "surround enhancement", "3D Mode" and crap like that, also within windows vista or win7 sound settings disable the "exclusive mode" settings if you have any. All these can cause issues.
Also don't set ur sound card to 24 bit and/ or "studio quality" just because you think it should be able to handle that, can cause plenty of issues as a lot of hardware and many games don't seem to handle that well and in those cases audio stutter, lag and higher CPU usage are likely outcomes, of course with no increase in sound quality.
The Problem with louder warping between systems and some cutscenes seems to be a general oversight as these are essential FMVs/ videos (even worse in low quality and with washed out colors, but that's another diligence effot bioware failed at) and not live-rendered in their engine so their sound lvl/ volume will be different and not adapted it seems. One of the many flaws and issues ME3 has, it really is rough around many edges, I blame Microsoft and their kinect money which distracted efforts where the game really would have needed more.
