He was killed in ME1, and didn't appear in my ME2 game...but all of a sudden he's alive in ME3? I don't get it.

The exact opposite happened to me. Wrex was alive and is now dead. Still waiting for an answer.

Hmmm... sounds to me like a plot flag got "flipped"... just speculating, but I think something similar happened in DA2. I suspect, if it is true, it will require a patch.

Doesn't seem to be universal. My Wrex is still very much alive. Maybe some other choice is interfering with it?

One possibility... have any of the users experiencing this issue used a third-party save editor on their ME2 character at some point?

I used a save converter to convert my 360 save to pc.

Then I'd say that's the #1 suspect. One thing you could try... do a Google search for "Gibbed's Save Editor" and take a look at your converted ME2 save. I haven't used it in a while, but there's a tab that will let you check plot flags. See if the 360 save converter missed something.
EDIT: If you do try this, please let us know what you find!

I've checked and there's a check next to Ashley killing Wrex(which is correct).

Checked my save too and it's exactly what it should be, ME3 even said that Wrex was alive when I imported my save.

I've got 2 imported saves one of each gender from ME2, but the one for the female shepherd thinks shes in a relationship with both Ashley and Liara, and keeps changing to either no voiceover when talking to ashley, or the male voiceover.
I think its just a case of flags getting messed up but once you're already playing the game and really far into it I don't know what you can do other than restart.

Yeah this must be a lag, why else would the game tell me in the beginning that Wrex is dead, but then bring him back to life?

Same problem as Greenhelm. Wrex was alive in ME2, now he's dead, replaced by Wreav. I did use a save editor on my ME1 saves. It worked fine in ME2, and ME3 says he is alive when I import the character.

Having the same issue. He is flagged 'dead' in Gibbed's save editor as well.

Ran into the same problem (Wrex was killed in ME1 and dead in ME2 but alive in ME3) and have done some digging.
Think I may have found a fix for this using Gibbed will try it and let you all know the results..

Seems to be working so here's how I killed off Wrex (Yet again).
In Gibbed's Editor for ME3 load your save file then:
Click on plot-->now select Bool Variables on the right side and expand it by clicking the [...]
This will bring up the Booleen Collection Editor. In here you will need to adjust a few of the numbers either between True or False depending on what outcome you want (Wrex alive or dead). To make changes click on the number you need to modify then change the Value displayed in the right screen. List of needed Values below.
If you want Wrex DEAD:
True:

If you want Wrex ALIVE:
True:
