...ended up dying when you confront her father and Oriana. I read people saying that if you warn her about Kai Leng then she will survive. That was definitely not the case! Has anyone else saved her? What did you do?

I believe you must continue your relationship with her also besides warning her about Kai Leng for her to be saved, but not 100% sure since I stick with Liara but the guide makes me believe this is the case.

Currently suspected conditions for Miranda's survival:

(1) She must be loyal from ME2
(2) Warn her about Kai Leng in the vidcomm conversation after the coup
(3) Trust her when she asks for Alliance resources in the control chip conversation
(4a) Have enough reputation to use the persuasion option in the confrontation with her father,
(4b) OR use the Renegade interrupt to wound Oriana, getting her out of harm's way.

Has anyone played a game where Miranda comes unloyal from ME2? I would like to know what happens then? Do you ever meet her before Sanctuary?

I'm currently playing a game that is a non-import, out of curiosity (or was, late yesterday). So far no contact, but I haven't advanced very far.
