1. I cannot install the game to my hard drive, and whenever I try, it says that it cannot read the disc after about 41%
2. The sound cuts out the moment Vancouver is shown.
3. Random textures are extremely poor.
4. The game just freezes shortly after the first dialogue option, if not during the scene with the fleet.
Is there anything I can do to fix these?

Been having the same problems. Everything worked fine the first time i started the game. I loaded up my save right after you see the kid in the vents and start to run through vancuuver and the sound cuts in and out and the whole game just freezes shortly after. Went back and tried to start a new character and the whole thing just goes to black screen right have the "a bioware production" screen. What the hell man?

take the disc out and see if there is a perfectly round scratch all the way around the disc. Thats what happened to me. played fine the first time, apparently there was a vibration or something that hit the xbox when i restarted and it put the scratch on there. taking it back to gamestop to pay the $3 insurance charge and getting a new copy.
