Uhm... I was able to bind reload to "T" in the demo (using ESDF rather than WASD), but I cannot do so in this game. What is "T" used for? And why the restriction...

T was supposed to be for text chat in multiplayer. At least, that's what it was attached to in the demo. Seems like they took out text chat and forgot to free up the key.

Anyone know where are the usersettings.ini or similar file is located at? To manually change keybindings (if possible)?

Looks like they hid it because they don't want us to change anything. We were able to cheat in ME1 and ME2 and bioware is ptobably not to happy about that. On the other hand, it allowed us to change insanse mouse sens in ME2 and it would be great now.

It's hidden in the Coalesced.bin, and it's hidden there because pretty much every crucial data is also in there. I mean, there was this tool that can change it, and there was data included on "the maximum level in Multiplayer" and the like, and that's just one of them. That was for the demo though, and I have not checked it myself for the actual game.
So they'll just hopefully fix this issue without having to mess with that file, as Origin might ban you for a modified file. Though who knows, maybe they wouldn't mind this change.

What I did was use Autohotkey. If you're interested, I can elaborate further.

I was just researching key remap options. Programs to remap keyboard will not work since they change registry which require pc restart. Or, in case of MS key remap tool, it will simply add a new layout that doesn't work in game.
So, the next solution was autoit or autohotkey. I prefer autoit, but sometimes it is way more complicated than it should be.
Is it enough to create ahk script like this?

Replace the green text with the path of your Mass Effect 3. This way if you click on it, it runs ME3 too.
For those who are unsure,
1. make sure that both the "t" and "e" are lower-case in hte script
2. "t" must always come before the key you wanna assign it to.
So for me, when I press "t" it's actually pressing "e".
If you're like me, and would like to luanch ME3 via steam, there's a slightly more tedious way to do it

I can also upload .ico if you need it. It is packed in .exe
