I heard that if you don't warn miranda about kai leng she dies later on in the game. I just had mordin, thane, and kelly die in succession so I kind of want to prevent it. When, exactly, do i warn her? I just got done with udina's coup and I already met her once on the in the citadel docks.

Yeah she survivied on mine but I don't remember ever warning her about Kai Leng. Edit: Actually I now remember. You talk to her a second time on the Citadel later and if I remember right you warn her there.
On another note, where'd you find Kelly? I didn't see her in my playthrough even though she lived.

I cant say for sure (its possible I missed it, though highly unlikely). However I broke up with her and never got the chance to warn her.

I believe Shepard tells her regardless (or at least in mine he did from what i recall) but if in doubt just go for the top right choice when dialogue comes up.

Just make sure to talk to her a second time. The second time you talk to her it will be via hologram in the Spectre place. BTW you can also save Thane and Mordin.

For Thane you need to do two things.
1. Make sure to have Captain Kirrahe alive, Kirrahe will stop the attack and he'll die instead.
2. Don't talk to Thane before the attack. Just avoid him
As for Mordin, he can be saved during the mission if Wrex isn't alive, Maelon's
data was not saved, which leads to Eve's death, and the genophage cure
is sabotaged.
