I've heard 2 diffferent conversations that indicate Kelly Chambers was killed during my first playthrough ... but her name won't appear on the memorial wall on the Normandy. FIX IT.

That was a sad, sad story. Met her on the Citadel and she was suffering from PTSD somthing fierce. If she had taken me up on my offer of service she would still be alive in the game. She served the Normandy SR2 with honor and integrity and deserves to have her name on the memorial in the crew deck. I was not happy when I learned of her murder and noticed that her name was not on the board. She was killed because she was loyal to Shepard pure and simple. She should be on that wall.

When you first meet up with her you need to tell her to change her name and appearance. That way when Cerberus occupies the Citadel they don't recognize her. Unfortunately she decides to dye her hair puke color for some reason,

Yeah, I could have kicked myself for not telling her that. But with the destruction of the Citadel she's dead either way, but that's metagaming.
