 I stayed Loyal the entire time, got to a point where it zoomed in on my bed where someone else should be when attacking Cerberus and nothing.

I talked to her, told her we were still lovers, but outside of that I got no options for much with anything of her.

Did you go paragon choices with her at all hospital conversations, and give her the Tennyson gift book? You should reach a point where she meets with you in a cafe on the Citadel and you can choose to affirm the rekindling of the relationship there. Assuming all that happened, the intimacy scene should play before the attack on Cerberus.

I gave her everything, never had any other options to meet with her that I can recall.

If there was, then it was far too hidden for me to find her, I checked every area of the station many times.

I don't recall the cafe, I did reaffirm it before I accepted her on my crew.
