Insanity is definitely easier than it was in ME2. (I imported a character from ME2 so I started at level 30.)
ME3 is ridiculously hard! Agree?

playing vanguard and except for the odd turret or bansee its not hard for me. I did carry over my ME2 character which helps.

I played through ME & ME2 on Casual. Then I started ME3 on Normal. My only problems thus far are as follows:
- Combat Engineers (They can all go to hell...)
- Turrets (I'm wondering if Infiltrators can "sap sentries"...)
- Banshees
- Shepard's inability to take cover for some reason
- Ally AI don't help sometimes
- Cover doesn't always do anything
I'm a Soldier with a Mattock V as my primary weapon. I have a Sniper Rifle for backup and my power is Carnage (which is maxed out). The SP is tough sometimes and the combat is really challenging. I even thought of setting it to Narrative but I'm too stubborn and want my traditional Mass Effect experience.
The REAL hard part is MP. With only a level 10 Soldier, you can barely do anything. Death comes quickly and your team is lucky to make it to Wave 9 or 10. I've never beat a MP mission. IT'S SO DAMN HARD!

Try not spending all of your points...
But, I am a HUGE fan of challenging games. The harder, the better!

Ah I'm looking at it from soldier and vanguard. I swear charge does nothing but get me killed anymore.
I have this same problem. It seems enemies are bunched together moreso than in ME 1&2. I'm starting to think that Charge is not the best tactic indoors.
And then you start using charge biotic detonations, and you will find yourself charging everywhere, once again
Liara - Singularity.
You - Charge those sons of ****es.
Next pack!

My soldier Shep with a Mattock V plus mods with Liara and Garrus just chainsaw through levels on Normal. Very satisfying.

I have one word:
BANSHEES.
Seriously. Whether it's single or multiplayer, every time I hear that scream it results in an immediate facepalm. ALL MY FRUSTRATION.

Uh... Up until this thread, I've heard the opposite.
Myself, played on normal - huge mistake. Died once - when I charged stupidly into a radiation wave. Haven't died in a fight a single time. Playing as an engineer and those banshees don't even come close. It's too easy.

HAHA! I ran into (dropped into) the same radiation. Insta-death. I had one more insta-death too, but I never died in combat playing as a soldier.

I am playing this as an Adept on hardcore and I find this to be much easier then ME1 or ME2. I am seriously considering just playing this on Insanity now for a challenge. I actually find this game to be a lot easier. Not going to lie.

Playing on normal and after 14 hours or so I haven't died once. I imported my level30 infiltrator. It's harder to camp in one spot but I definitely wouldn't call the game challenging. I was actually surprised I'm having it this easy.

I am still on my first play through import shep vanguard. I started on normal but found it way to easy. Eventually I had to bump it insanity. Running with the eviscerator shotgun only my recharge is like 160%, even with this on insanity I feel a bit overpowered because the recharge on bio-charge is so fast. Really the only mob that gives me issues is banshee's, and they only started to on insanity.

I am playing as an imported adept that i started in me2 in normal mode
My tips for those having problems:
Remember the code of a leader in combat(in a video game): always know where your people are, and Tell them where to be.
- what i mean is, you move 20 or so feet forward, then move your people forward from you (leap frog style). If you don't they either won't move or will take a different route, That is very bad news.
Before opening a door, position your people to the left and right of the door then open the door. After the door is open, move them in first to cover, then enter the room. (if you just run in, they may or may not enter. then you are alone.)
After you are in the room, and they are in cover, then leave them there and explore the room a little. To see if the enemies will be tripped or activated if the enemy shows up. then find cover to support your people who are still in cover, and make sure they can cover you.
- translation: don't stay in cover to far away from your people.
Not sure if that will help anyone who is having problems. but hopefully someone can use it.

I died 7 times during the game. 40 hours in and almost done. Normal difficulty.
Not that hard for me. But I can only imagine playing on Insanity... D=

ME3 is very simple on insanity, not sure what you are doing wrong; it was way harder in me2. Like, throughout the whole game, I died like 7 times, and all of those times were from the fact that bioware linked majority of the actions to the A button.

The only creatures I am having problems with are the cerebrus ninjas.. actually i can't remember what they are called you run into them on the citadel when fighting cerebrus about 1/4 or 1/2 through the game. they teleport. i finally gave up on shooting them and would keep tapping shift (on the pc) until the game was paused with them targeted, then i'd use my power and the powers from the other squadmates on him.made them a little easier.
the banshees are annoying, but not bad.

First play through I did it on hardcore, and never had a problem except for 1 part on Thessia.
Have died now and then, but thats because I play it like rambo, charging in with my shotgun and attempting to melee everything when i'm out of shells.
Just found it extremely annoying vs banshees / leng as I was pretty much running backwards the entire time pummeling them with bullets until they went down.

Triple chain overload (Sentinel Shep with Garrus + Kaidan or EDI) rips through most things except Banshees and Brutes, so I supplement with Warp, AP ammo and modded weapons. Still had difficulty with a mission with 4 banshees simultaneously appearing but realized the combat would end if I just rushed the objective. It's so satisfyling to electrocute Nemeses and Phantoms before they have a chance to do anything.

As far as I can tell the difficulty doesn't come from ridiculous bonuses this time around, which is a welcome change. The AI is smarter and maybe the level design helps too. I noticed enemies circling around me and attacking me from behind. Also, grenades prevent you from just staying in cover indefinitely, although it's usually easy to dodge and go back to the same spot

The only tough enemies are banshees, they always kill ALL of my squadmates at least 2 times. At least husks are easier to kill, and there are not any scions anymore. Also the guardians are annoying, had to take liara in every mission(i am a soldier, also she always goes near atlases for no reason when i order her to get to cover). All of this on normal
Banshees are the new scions.
