I think it was the correct play. Being that it is the endgame, might aswell crank up the difficulty so once you get good at it you still have a challenge.

The only thing that irks me is that my squadmates don't seem to realize that grenades hurt you as they keep standing on the same spot waiting for grenades to explode.

Note: I don't have very good game skills so I figure the folks that do have good skills need the higher difficulty. Also there was almost always an approach to each fight which turned it from impossible to merely "difficult" IMHO.

The best idea is to use combo powers. A sniper rifle with the concerntration and smoke filter mods plus freeze ammo is brilliant for exploiting combos. Flash freeze plus incinerate causes a lovely explosion. Warp plus concussive shot is also quite fun. My personal fave is lift grenade followed by a rapid sniping session. A Mob helplessly floating upside down plus a direct headshot is just SOOOOO satisfying
I would suggest experimenting with the squad you use and try and plug any gaps in Shepards arsenal or think about what powers would compliment yours. If you are an all guns blazing type then for instance Tali can run interference with her drones and Liara can keep them debuffed with biotics. Try and remap your powers to quick buttons depending on the situation and map L/R on the D pad to something useful like stasis/warp etc. A banshee isn't quite so tough held with maximum stasis. A few well placed salvos from machineguns with disruptor ammo soon rips through the barriers then some sticky grenades and salvos of incinerate takes care of the armor.
Also how many weapons are you carrying? Don't forget anything over 2 or 3 will actually give you negative recharge on your powers. I`m quite happily banging out incinerate approx every 3/4 seconds which panics enemies and rips armor. My power recharge is +162% as I'm only carrying a Sniper rifle,machineguns with the light materials mod and a backup pistol. Also pick a USEFUL bonus power in the med bay. IMO slam is the best. It hits approx 8/10 mobs and bounces them up a good height in the air. Once they are up there hit the power wheel while they are in your sights and pick 3 powers that will hit them at the same time. If the triple salvo plus the damage of being smacked into the floor doesn't finish them then I dont know what will.
Next upgrade your weapons to Mark V as fast as possible and I would say mod ALL your weapons with +damage and +accuracy. Apart from sniper rifles which imo should have +concerntration (for slo-mo sniping) and the smoke filter.
Just for ideas I would say the following (so far) are the best to use in terms of rate of fire/accuracy/damage which I beleive are the three KEY stats.
SNIPER RIFLES ASSAULT RIFLES SMGS SHOTGUNS PISTOLS
INCISOR (3 shot salvo) PARTICLE RIFLE HORNET GRAAL SPIKE PREDATOR
JAVELIN (shoots through GETH PARTICLE TEMPEST THROWER ARC PISTOL
cover) RIFLE (INSANE ROF) CARNIFEX
Obviously this is all subjective but I fimly beleive ammo increase/thermal reduction/melee damge/shredder mods etc are a big fat waste of time. There seems to be no percentage in a large ammo clip if you could replace that with a mod that does 25/30% extra damage per shot. As for 35% armor penetration at a 50% loss of power ?? Pfft ... If you haven't got warp/freeze/inferno or armor peircing ammo on your weapon then BOY have you made a bad choice of character.
Hopefully you can take this in the friendly spirit it is meant. I am GENUINELY trying to help people who are maybe going `ARRRGGGGHHHHHHHH` and running round in circles. I really like ME3 and think it is a worthy finish to an interesting and sometime slighty bumpy trilogy. I certainly will be hard pushed to think of any other game character that I have invested so much time in and seen the tragic results of what I sometimes think is the `right` or `moral` choice.
Anyhoo I hope I have helped and maybe given people something to think about.

The enemies are far weaker than the ones in ME2. The Collectors were far more annoying. Maybe its because i already played MP alot on the demo, i'm used to the cerberus enemies. Only time i remember dying was that one mission with the banshee spams. one crept up near me while i was blasting another.

The Bonus powers are unlocked by having enough converstions with each team member. They dont replace any of your other powers and just fill a spare slot on your power wheel. You pay 5000 credits to buy it and then just invest skill points as per normal. You can change the power by paying another 5000 but not sure if you lose the invested skill points in it or not.

It seems a lot harder to get shepard to take cover than ME2 on my PC did; it's very hit or miss whether he'll just roll away or stand there doing nothing. I'm really bad with a joystick, though, so I'm probably just not getting shepard into position accurately enough before hitting the all-purpose action button.
Still, the AI seems much smarter and they've got some pretty frustrating tricks. Very fun game, though, once I get better skillfully taking things out will be immensely satisfying.

Insanity
Shepard - Soldier with full auto rifle (ALWAYS FULL AUTO)
Garrus - Sniper duty
Liara - Biotic duty
Essentially combat is waiting for Adrenaline rush and popping that and then staying ducked until it recovers. It's otherwise impossible IMO (as a soldier) to do anything else. One soldier can mow you down in a few seconds if they get a good attack on you on Insanity mode.
I'll admit, I had to cheese through some of the areas to beat em. i.e. when you save Grissom Academy, some of the room clearings were harsh. I just stayed in the corridor and lead the stupid guardians/centurions into the corridor. It was just impossible doing that area with an ATLAS launching missiles at you with Guardians dropping from the sky.
But yeah it's hard, I probably have over a few hundred deaths/reloads. But I like the challenge, it made up for the horrendous ending.

Witcher 2 was really easy? Lol, you're the first I've seen to make that claim! I know they were planning on releasing a patch to make normal easier, maybe you played after that.
