I completed Mass Effect 3 without ever encountering either of them. They were never even mentioned. I'm 100% sure that they survived in my Mass Effect 2 import. I completed it a day before ME3 was released.
I know where to meet them on the Citadel but they were not there. I took the earliest opportunity to find them.. Still gone.
Why?
Am I to complete Mass Effect 2 from the start again just to get these two characters? Understandably I don't feel like doing that. I feel like I missed a part of the game.. however small. I may never experience these two characters in Mass Effect 3.
Kasumi and Chakwas missing from ME2 import.

I found them both the first time I went to Citadel too. The doctor was talkjing to Ashley's doctor in the waiting room, and Kasumi was found by talking to some Salurian specter outside of C-Sec. No clue why they wouldn't show up in your game. For the record, I recruited Kasumi after beating ME2, and saved the doctor during the final mission of it.

I never got Kasumi to show up on my first playthrough. Starting my second now and I already visited the Citadel once (when you automatically go there after Mars) and there was no quest.
Dr. Chakwas (provided she survived ME2) is down in the hospital on the Citadel talking to Dr. Michel from ME1. They are on the left side of the first long room, where you meet Thane and Jacob (later on). Chakwas will ask to come back but you can turn her down and take Michel instead if you like.

I don't think Kasumi was there the first time you get to Citadel (when you only have access to half the station), but she was definitely there the first time I went there and had access to the Commons and Purgatory and other areas, soon after that. You might also need to get an email first, because the instigation of that mission is another Specter that has an issue with some Hanar.
