Boom! Shepherd get's knocked down by the big attack from the Reaper in London.
He staggers to the Beam and gets transported to the Citadel, does his thing, picks which path to take blah blah.
End of Shepherd.
HOWEVER!
The DLC makes it all one concussed hallucination/nightmare. Shepherd wakes up to find his squad still with him in full health. They all stepp into the Beam and go and kick arse.
They win. The End.
I know it's a cheap, stupid idea that shouldn't be taken seriously at all, but in my mind this "daytime TV drama ending" Is far more satisfying than the one we all get.
'nuff said.

I personally have 2 ideas of fixing ending situations:
1) Leave the curent endings as they are, but add a number of other choices leading to different resolution.
2) Make a Fallout 3 style DLC, which continues the game after the final mission and allow to resolve the fate of various characters.
