In an Italian forum, the Admin knows a game tester (not EA, i think) and he said that they spent lot of hours testing the game.
Sure, i'm disappointed because of the bug, but consider that the debug version used by the magazines works fine. It seems a problem of disc printing or a corrupted file.
Let's wait for an official answer by BioWare.

Happy that you do know how a game "should" be tested but that doesn't mean that every company does that, for what I know Microsoft could test every version of a game for days, I don't know what they do, but I do know that a game big as much as ME3 requires a lot of time to be completely tested, weeks maybe, EA only has few localisation testers, and from what that friend of mine told me they usually don't test full games, specially if the game is this big and requires so much time to test every gameplay\\story option.
EDIT: To be clear: EA localisation testers have to test all EA's games, which are a lot, that's why they do not spend so much time on every game, even working 8 hours per day 6 days a week, EA has tens of games in development right now.
EDIT2: Please do not confuse "game tester" with "localisation tester", those are two different jobs, a localisation tester do not test a full game, that's the game testers job.
