 When on Firebase Dagger in Round 5 or 6, during a terminal hack on the far west side of the map, I took cover while hacking and fell through the map. Was unable to participate for the rest of the game. At one point a Banshee somehow killed me while I was falling in a "void" below the map. When the next round started I re-spawned in the "void" again.
