HOW did she die? And how did SHE survive?

This thread is about one character mysteriously dying without explanation...and, more glaringly, one character mysteriously surviving their own death...and then teleporting.
In ME2, I quasi-romanced Jack. Not the full romance, just the "quickie." I had decided to fully romance her in ME3, given the chance...but I never got that chance, because she was dead. She survived my ME2 suicide mission, but randomly showed up as a...husk? Whatever. A bad guy, because she was suddenly and unexplicably dead.
I looked it up, and apparently this happens if you don't save Kahlee Sanders in time. Okay, no problem. Love it, in fact. My favorite thing about ME3 is that actions have consequences. But it wasn't explained. This is the dialogue:
(no indication that it's Jack, other than the character name. After killing her:)
EDI: I think that was Jack.
Shepard: The Reapers will pay for that.
That's all I get, after having to kill the husk of a woman I had sex with and intended to romance? At LEAST give me a cutscene.
But okay. I could live with that. I still considered ME3 to be the greatest game of all time, until a dead character mysteriously showed up at the ending.
Having lost the opportunity to romance Jack, I settled on Ashley, despite not romancing her in ME1. I'm glad Bioware gave me this option, and I liked the storyline. When it came time for the final battle on Earth, I decided to take Ashley (my love interest) and Garrus (my best friend, since Mordin died curing the Genophage). I was hoping for an ME2 cutscene of desperately clinging on to someone's hand as they dangled over the ledge. Something like that. Or we all die, or whatever. We'd be together.
Then came the laser beam, and I was the only survivor. AWESOME. Loved that my love interest and best friend were dead, made the consequences even more meaningful.
Then the final cut scene, Joker trying to outrun the burst, failing, the Normandy being shredded. Great! More consequences!
Then the scene with the crashed Normandy. Joker gets out...followed by Ashley...followed by the Prothean.
WHAT IN THE WORLD?
How did Ashley survive the beam? Even if she did survive it, why didn't she follow me into the Citadel? Even if she did survive AND decided "f that" and ran away, how did she get back into the Normandy? It was outrunning the blast, it didn't have time to make a pick-up. Looking at other videos on Youtube, I'm thinking it automatically shows Joker with your ME3 love interest at the end. Why? Joker has his own love interest, which would have been more rewarding to see. (Admittedly, he'd have a hard time continuing the human race with EDI).
So this is my biggest disappointment: A lack of closure for most of the characters, and the closure I did get forgot that one of the characters HAD DIED.

There's actually (slightly) more to that - in the room before you get to Jack there's a datapad that has an audio recording of her being tortured/converted.
How did Ashley survive the beam? Even if she did survive it, why didn't she follow me into the Citadel?
She didn't enter the beam - I had Liara and Kaidan in my party and I'm fairly certain their bodies were on the ground well back of the beam when Shep stumbles forward towards it. I thought that ought to mean they died, but I suppose the story is that they're picked up and healed in that in between period.

Then when I chose Synergy ending, both Ashley and Liara(after Joker and EDI) are seen exiting the Normandy. How did they get teleported to Normandy???

Really? Because I got the crewman I played with most and my LI, as did the two friends I have who played it. If they're random, thats kind sad.

Far as I've seen the ending cutscene charaters are your active squadmates on that last mission. With a Liara LI and Garrus, this is what I got:
- "Merge" / Blue ending, Joker steps out, Liara stands next to him and Garrus steps out as the camera pans back
- "Synthesis" / Green ending, Joker and Edi step out and semi-cuddle, and Liara steps out as camera pans back

Joker, EDI, and Ashley (my Shepard's LI) were the ones that stepped out of the Normandy. I also replayed that ending (because the game froze during credits) and it was the same 3 again (Joker, EDI, and Ashley).
I highly doubt it's random.

I guess it could be random, but has anyone had their ME3 love interest NOT show up in that final cut scene? For me, it was Joker, love interest, and the Prothean that I couldn't care about in the least. I mean...yay, the guy who said HE WOULD COMMIT SUICIDE after the final mission is one of the three who survived! I wonder if he shows up for everyone who bought the DLC.

Agreed. I could MAYBE buy the other squad members somehow escaping...but not the two I ran into the beam with!

OH! And speaking of no-name crew members, when I'm giving my final speech to the crew (on Earth), there's my whole team...plus one random human in a beret who sticks out like a sore thumb and is even blocking my view of some of the main characters. Who the heck invited that guy?

It's not. The game keeps track of which squaddies you talked to the most and had the best relationship with, and the ones that show up are the ones who had the highest rating. Case in point, Liara was my Shepard's LI and she alone stepped off the Normandy with EDI and Joker (I took Garrus and Ashley with me on the final push).I highly doubt it's random.
And, do the corpses of your two companions you take with you on the final push show up at the Conduit? When I beat it the first time I was too busy not getting my face gnawed or shot off to have a look around.

Huh. I got Joker, Kaidan, and Garrus - and I'd hardly used Garrus at all.

I tended to use Liara LI and Garrus for most missions, for the nostalgia factor. Had them both show up with Joker for one of the endings.

Nope, I'd had Liara and Kaidan. No idea why he showed up for me. I'm not bothered by it or anything, it just seems random.

UNLESS you have the Prothean DLC. Then it shows Joker, Ashley, and the Prothean, which is bull****, because who cares about the Prothean?
