Is there a way to do this? Miranda was my Sheps LI in ME2, but I changed to Cortez in the final game. Is this why she died or is it unavoidable? When Shepard and her are talking as she dies and he says,
"I've never met anyone like you, Miranda."
Miranda, "No, you haven't"
The waterworks started here! After Moridin, Legion, AND Thane--this really just killed me.

No idea how I did it but she didn't die for me.
She mentioned something about me warning her about Kai Leng and being able to prepare.

When she wants to talk to you on the Citadel the 2nd time in the Spectre Req. office, that's when you warn her.

I dunno, I was a femShep and barely talked to her in ME2, but I managed to save her. I read somewhere that if you manage to warn her about Kai Leng, that's what saves her. And I did talk to her in the SPECTRE offices on the Citadel on that 3rd terminal that never lights up. During that chat, I got the option to tell her about Kai Leng.

Damnit! >_< I really am not emotionally ready to replay the game, but I might have to now just to "fix" my mistake. Was it a conversation option, and is it after Thessia that you can warn her?

The same thing happened to me. I've heard that she dies no matter what if you break up with her. I think this is true because my Paragon was maxed out and she was auto-warned about Kai Leng in the Spectre center.
It's either unavoidable or a bug.I couldn't Paragon The Illusive Man at the end either. Even though I was maxed out since half way through the game.

I don't remember either. I generally checked every nook and cranny of the Citadel after every mission and session of planet scanning trying to complete those darn fetch quests to try to up my assets, so I was always making a run through the SPECTRE offices to check for stuff, and one time that terminal was lit up so I checked it and got a vid convo with Miranda. I honestly don't remember when.

You get an e-mail telling you to meet her in an apartment on the Presidium Commons.You must do this before Horizon towards the end of game.

If you break up with her she dies.
I gave her Alliance resources. Warned her of Kai Leng. Had her Loyal in ME2. Rep was maxed out. Paragoned her final encounter.
Still dead.
