Not sure if anyone has pointed this out. Just encountered a glitched in the Citadel Embassies. Was walking around, and ended up falling over one of the rails, into the pit area.
Just thought I'd mention this so that a fix can be included in a patch. Nothing huge, I can load my last save still.

I haven't had this happen at that location, but I did have a similar bug happen:
During one scene (no spoilers), you're supposed to jump from a moving elevator to another moving elevator - I forgot to press A and literally just walked off and started a never-ending fall. To fix, I reloaded (it autosaved immediately before the elevator jump) and it worked. Didn't think about trying to duplicate it - I apologize.
