The 'kuring Keprals syndrome"... I'm not a fan of the idea, because it makes it too similair to Tuchanka plot, and would make Shepard some kind of galactic paramedic, whole game taken into consideration. Also, Kepral Syndrome's scale is not 'genophagian'; survival of the species doesn't depend on curing it.
Also, I think that renegade choices should have some form of reward, for example more assets in any form, and paragan choices should have some drawbacks. In other words - choices should be hard to make. You can save Mordin Solus, but you must sacrifice whole Krogan species to do that, and this is actually a hard choice. Of course in the Eve & Wrex dead scenario, otherwise the paragon way is more obvious.
- If you choose to keep the cure and save all the drell on the DLC: You got more assets.
- If you choose to destroy the cure on the DLC: You got anything XD
that's what I'm talking about.
As for the DLC structure and form, I'm all in.

About your question: You are right, that can be a problem, but we can change the person who sends you the first message. If Cerberus is attacking the hanar, maybe the Alliance has discovered their plans, and Hackett can send you the message, asking you to go and help the hanar. When you land on Kahje, then you can meet Kolyat.
About the cure, it's only an example, can be a transplant too. Can be anything, something to save Thane or extend his life. I said Kepral's cure in the first place because is a reason for Cerberus to go to Kahje. If Cerberus is involved on the mission, we have a reason for go there and fight, not only for Thane. This way more people can be interested on a DLC like this.
And about the choices... maybe save only Thane/save all the Drell? Or if you want to save Thane, you must sacrifice someone else (not Kolyat please:crying:).
Anyway, the details can change, but the structure is the same, a DLC before the Citadel Mission, with more content, and an option to save Thane. But not focused only on Thane fans (I don't think Bioware wants to do that).
