So I'm just about done with my second playthrough.. and a few things don't quite make sense to me.  I'm wondering if I just.. missed something?  Completely overlooked a PDA message or.. some dialogue.. I dunno', either way:

1) Is it ever actually said who the "traitor" is on Sur'kesh?
Various team mates comment upon it as you're saving the krogan female, that "Obviously there's a traitor 'cuz Cerberus is here as just the right time" yadayadayada.. Was this ever actually answered?

2) Benning - Cerberus denies that they're the ones killing civilians on the planet.. Shep and Hackett figure they're lying.. and yet at the end, Hackett was all "Well.. killing humans isn't something Cerberus does.. so maybe it wasn't them."  Is it just an assumption that yeah, it was them 'cuz of the indoctrination, or was it something else? 

3) Greyson - I read the book, so I know about him.. but how does Shep?  Anderson mentions him more than once, but Shep never asks who Greyson is.
