I've had several instances in dialogue & cutscenes where the camera points at nothing or actors are standing oddly.  When I say oddly it is like a piece of a body is pinned to a specific pose but it doesn't fit with the rest of the body/scene.

1.  When talking to Engineer Adams for the first time he is talking offscreen and the camera is showing me nothing but bulkhead.  This occurred around the middle of the conversation.  Afterwards, when onscreen Adam's eye's and head were pointing away from Shepard, but his body was pointing towards.  The neck wasn't fully 180, but he looking away from Shepard while talking.

2.  When talking to Liara in her office for the first or second time (that long dialogue around the beginning) on the Normanday she would look right, the camera would point left and I'd get a side profile while she talked.  However, her eyes would be pointed at me.  Then I got an overhead view and Liara was looking right, Shepard was looking left and their bodies were facing each other.

3.  After the Udina scene on Priority Citadel I was playing with Javik and after he walked off screen the camera is focused on a railing for several seconds.  There was nothing there.  I don't know if it was supposed to be showing Udina or something else.

4.  When meeting Jacob & Brynn if Liara is with the party Jacob will talk to her and she will repsond.  However, when Liara talks there is no one on screen.
