I know in ME2 you buy a upgrade for it, but as far as I can tell the only way to remove facial scars in ME3 is by not being a renegade. Do I have to just be a nicer Shepard to heal my facial scars?

Eventually, Dr. Chakwas will give you a sidequest to meet some refugees on the Normandy. If you help them, I'm pretty sure you can talk to Liara's robot (Glyph) and he'll give you the option to heal the scarring or take some other benefit.
