This is a small theory of mine.

If you talk to Liara enough, she'll eventually want to visit you in your cabin. Once there, she tells you about how she's been working on some time capsules and wants to include Shepard. She also says that she plans to have them land on several different planets. It is possible that one of these time capsules landed on the snowy planet where we see a grandfather talking to his grandchild. The grandpa talks about space flight and what not, while the kid asks for another story about "The Shepard". No, they are not descendants of the Normandy crew. If you want to talk about how the Normandy ends up on the jungle planet, go to the main "Was it a hallucination?" thread. They explain that one quite well.

TL;DR: The grandfather knows about Shepard because of Liara's time capsules.
