Zaeed didn't survive the suicide mission on my ME2 file, but when imported, Zaeed's name isn't on the memorial wall. Is this a glitch or an oversight?

I have that same problem, in one of my ME2 playthroughs, Zaeed didn't survive, but his name is missing from the wall (however, I did sacrifice Jack and hers is present)

Kasumi died in my ME2 playthrough and she wasn't on the wall, either. But the other people who died on my ME2 playthrough (Mordin and Miranda) were there. Glad I'm not the only one. Little stuff like that bugs me.
