So i just did the Horizon mission, and at the end Miranda died. I choose the Paragon option where Henry let Oriana go, but Miranda used her biotic powers to push Henry out the window, she was probably injured by Kai Leng or something. Anyway is Miranda supposed to die? Or can you save her? If so how? Maybe you need to choose the renegade option?

Btw, did romance Miranda in ME2, but choose Ashley again in ME3, so had to say things have changed to Miranda the first time i saw her on the Citadel, maybe that's why? She will only survive if you romance her? Or?

You have to warn Miranda about Kai Leng in your earlier meeting with her on the Citadel.

its possible to save her. i think kai leng is mentioned in passing one time or something and she heeds a warning. i thought she was gonna die in my first play through but she survived

This is what I've heard are the things you have to do to save Miranda:

1) Have her loyal from ME2
2) If you romanced her, stay with her; if not, see #1 -- if you reject her, it could affect her fate
3) Warn her about Kai Leng -- this happens automatically, I'm assuming because you met #1-2

Hmm well, then i have no clue, can't say for certain i warned her about Kai Leng, but i don't see why i shouldn't have done it, I've gone Paragon all the way.

She was loyal in ME2, and i romanced her, but said no in ME3 when i first saw here, because i went for Ashley instead.

Edit: On the other hand, she got pretty upset when i met her the first time on the citadel, as i said things had changed, maybe i choose the wrong time to say the relationship was over, maybe that was the error.

Yeah, I think that may have to do with her dying. I suppose it's meant to be a "consequence" of you breaking her heart. I guess taking that path means that you, for some bizarre reason, don't connect the dots and decide to warn her about the assassin.

If it helps, her response was something like "Kai Leng is still with Cerberus? I better watch out." or something like that, meaning, she'll be more aware of his attempts.

In regards to being Paragon/Renegade, it doesn't matter because Shepard says it automatically when you turn on the console in the Spectre room.  It's just a "meet x and x" = extra line of dialogue.
