The beam hit in front of Shepard, maybe we can all just pretend or believe that Shepard was knocked unconscious at that point. It could all make sense if you stopped and thought about the endings that we got..
Ventboy or Citadel kid - When the boy died back when Shepard was leaving Earth.. it scarred Shepard to the point that he has dreamed and thought about him ever since so it would be less than surprising if the kid appeared in a larger role in a bigger dream.
Control Ending - This can be easily explained by the Illusive Man and how every time you talk with him in this game he is always talking about how he "will control the Reapers", so deep down in Shepard's brain the thought is there.
Destroy Ending - Not only did Anderson keep talking about it but so did Shepard. Since Mass Effect 1 this has been "the ending" that everyone including Shepard was gunning for only with less "mass relay" explosion and geth being killed and all that fun stuff.
Synthesis Ending - It's the obvious 'peace' choice.. synthetics.. organics living together having babies. Throughout the game you not only help secure peace (based on your choices) between the Quarian and Geth but EDI and Joker start a relationship of their own. Maybe in Shepard's mind he was just hoping that peace between the races would last and that EDI and Joker could find happiness or something.
Anderson dying - Shepard has been worried about this particular thing ever since he left Anderson behind on Earth at the beginning of the game. He regretted leaving his friend behind and it's probably at the front of his mind so it's not unsurprising that he thinks about Anderson potentially dying and has nightmares about it.
Illusive Man confrontation - This entire confrontation was a mix of classic Illusive Man (humanity first!) and the confrontations with Saren (they can't control me!) from Mass Effect 1 right down to the being able to convince him to the point that he kills himself.
So if you think about it and try hard enough you could just pass the whole thing off as Shepard wanting to believe he had succeeded when he failed in his final moments or Shepard being knocked unconscious and the whole "ending" was just a dream allowing for them to release ending DLC later..
