The ending is crap here are my suggestions.

1) Lowest Military Strength:
Shepard makes it to the Citadel but the Crucible is destroyed because the fleets are too weak.
The fleet is decimated and the Reapers control the space around the Citadel. Shepard, dying, tells the Normandy to run but Joker and the crew ignore this and attempts a rescue. The Normandy is disabled (al la ME2 intro.)
Shepard and the crew say their goodbyes over the com. Shepard has Liara update her time capsule boxes with the: Cruicible blueprints, a warning about the Reapers and their story via the extranet for future cycles. Everyone dies, there's a montage of all the races falling to Reaper forces followed by a future race finding the box and decoding it.

Also if Sheps para/renegade score is high enough he/she can use it order the Normandy off. In which case they say their goodbyes. Shep dies and the Normandy crew die fighting the Reapers in the montage.

2) Next Lowest Military Strength:
Same as above only the Normandy can recover Shepard because the fleets aren't as badly damaged. Shepard and the crew die fighting the Reapers. Everything else is the same (Future race finds the capsules etc...) but the montage is slightly different. Basically Shepard gives a final speach telling them how much they accomplished and how much they meant to him/her before they all die in a final battle to hold the last enclave of resistance.

3) Moderate Military Strength:
The Crucible makes it to the Citadel but a key component is damaged (or maybe overlooked because the plans weren't advanced enough yet. Remember these plans were developed over several cycles.) There's no "targeting system" the Crucible will destroy ALL advanced life and technology in the Galaxy.
Shep makes a choice:
A. Call for Evac and fight the Reapers conventionally. See ending 2.
B. Wipe everything out. Shep warns Normandy first, so Liara updates her time capsules with Shepard and the Galaxy's story for the next cycle. Note: this wipes out the Reapers completely so future races don't have to worry about it.

Everyone's dead so no montage about the crew (something like this happens over the com before Shep pushes the button.) There's a scene of a future race finding the capsule and erecting a monument to Shepard, the crew and all that.

4) High Military Strength:
The Crucible makes it to the Citadel fully operational (no damage or some scientist realized the thing would need a targeting system and makes one.) The targeting system has a problem however, the Crucible will wipe out all synthetic life. EDI says she can reprogram it but it will take time. This risky since the Crucible is exposed and even if it survives, doing so will cause significant causualties to the organic fleets protecting it. It will also cost lots of lives in the ground war. Shepard can wait to try to wipe out the Reapers only (paragon) or go immediatly and wipe out all synthetic life (renegade.) If Shep goes paragon his/her wounds become mortal before the decimated friendly forces can reach him/her. He/she dies in a hospital after saying goodbye to the surviving crew (dies holding LI's hand if there is an LI.) With Renegade he/she survives their wounds.

If Shep dies and is a male romancing female LI or any shep romancing Liara: The LI is pregnant. She informs him/her of this on the deathbed and then cut to montage.
During montage Diane Allers interviews survivers 15 years later for a documentary on the Reaper Wars. This gives closure. If Shep is alive; there should be special attention given to Shep and LI building a life after retirement. If Shep is dead; the focus should be what Shep was like with the crew getting very emotional. The future race is shown during first contact with the council races several hundred years later. When the council rep is asked about their ideals by the young race, he/she responds with "Let me tell you about our greatest hero... Commander Shepard. <insert first few words tailored to your Shep's background like "She was a spacer born to a military family. During the a major military action ... fade out.) Also, during the interview part; if Shep dies then al-Jilani is interviewing Ashely (or maybe Garrus if she's dead? IDK I can't see Kaiden doing this) and Ash punches her in the face when she asks a loaded question about Shep. If Shep lives well... he/she can do it his/herself.

5) Epic Military Strength:
Same as 4 only Shep lives with both Renegade or Paragon options.

**Edit** Also, in a rosey rainbow filled world, the FULL epilogue (Shep saves galaxy, everyone recruited and most survive) should be like 30mins to an hour. I could understand if Bioware wanted that to be DLC since it would be really expensive to produce (alot of voice acting to cover all possibilities and such.)
