Shepard awakens after being shot by
Harbinger after being revived by a medic. It's still night but
Shepard's now in what remains of a hospital. Whoever else Shepard
brought with him is in the building as well. Anderson mentions that
that the rest of Shepard's squad is now MIA, if his/her LI was there
he'll mention, "I'm sorry for your loss. I heard that the two of
you were close."

Captain Bailey is still in contact
leading a resistance on the Citadel but is losing badly. The
Crucible was also attacked by Cerberus but the Alliance is still in
control. Worse yet, Shepard's body has Reaper tech in it that's
consistent from the wreckage of Sovereign and Anderson won't allow
Shepard to fight the battle out of fear of
indoctrination. 

Harbinger puts a stop to that and
attempts to control Shepard through his implants. This naturally
fails and Harbinger blames Cerberus, proving that Shepard was the
first of TIM's plans to control the Reapers and was selected for the
Lazarus Project because of his exposure to the Reapers and was
already becoming indoctrinated after defeating Sovereign. Still, the
Reapers' control over Shepard is tightening (hence the dream with the
Catalyst) and there isn't much time left for Shepard before he
becomes yet another Husk. Also, the reason why he was able to
rally so many around himself was because they were being
indoctrinated by Sheaprd's implants. Within a few more hours
everyone but Cerberus will be controlled by the Reapers.

Cue
boss fight against Harbinger. Anderson goes down during the fight.
Tells Shepard to tell Kahlee on the Crucible that he will always love
her. Shepard then gets picked up by Joker in the Normandy
and gets taken to the Citadel. Most of the crew was left to fight on
Earth with the exception of Joker and EDI. Unfortunately a Reaper
beam cuts through the Normandy after dropping Shepard off and the
ship explodes, killing Joker.

Shepard then assaults both
 Husks and Cerberus forces who are fighting each other for
control of the Citadel, discovers Bailey who's dying and says
that it looks like he'll never see that retirement he
wanted.

Eventually Shepard reaches the same room where he
fought Saren. TIM stands before him and explains how he was exposed
to Reaper technology and knew about indoctrination for years. Shepard
was the cure. Unfortunately, TIM now wants to use that technology to
indoctrinate the Reapers to use as weapons against anyone who
threatens humanity and plans on sparing humans while allowing the
Reapers to harvest everyone else. Shepard can the choose to
allow TIM to become a living god in control of the Reapers or he can
choose to fight.

Shepard then has to fight TIM's bodyguards,
including three Atlas mechs and five Phantoms while TIM puts up a
protective shield. Shepard then has to choose between using TIM's
device to control the Reapers himself or destroy the Reapers for
good. If Shepard controls the Reapers he has to choose between
controlling them for good and becoming the master of the Reapers or
just delaying them for another 50,000 years but going on living a
normal life. Or Shepard can destroy them. If Shepard had enough war
assets (5,000), then the Crucible will only target reapers,
otherwise it'll hit all the mass relays and all reaper technology
which includes Shepard.

War assets determine who survives with
3,000 keeping the ME2 squad alive and 4,000 keeping the ME3 squad
alive. If Shepard dies then Hacket delivers a eulogy at his funeral.
If his LI is also still alive then he/she will be standing over his
grave and possibly be shown to be pregnant. If Shepard lives then
he's shown talking to the rest of his crew/other important characters
at a celebration regarding his appointment to the council. Shepard
then gives a speech on his appointment.
