This is the "I charge while dying, and later (sometimes after several waves) start flying/falling through the map" glitch.  It has been noted in numerous threads in the Multiplayer forum (links and detailed descriptions of the bug included below).

I have consistently replicated this bug with Human, Asari, and Drell Vangaurds.

How long before Bioware usually acknowledges and/or fixes gamebreaking bugs such as this?

Same here keeps happening randomly , i first thought it was because of lag but i can watch spectator mode with no problem
