I told her Kai Leng was with Cerberus from the Spectre terminal, used the Renegade choice in the hold-up, but she still died. From what I have heard, that should have saved her. What gives? This is a bug?

As I understand it, her survival keys off if you got her loyalty in ME2, but I could be mistaken.

I had her loyalty. :-P And I didn't cheat on her, either. (Never even was with her)

Ah, I never met her in her apartment. I got an E-mail from her saying to meet her, but after I looked around and didn't find her, I just guessed that was about an earlier encounter.

Do you have to give her the Alliance resources as well to save her? That is the only thing I didn't do, and I didn't think it mattered; the scene stayed the same regardless of whether I did so.

yea it is a bug

i got the same thing, i warned her, talked to her etc.. etc..

everything was ok

but in the scene she have already been shot

stupid game, have to use savegame editor to correct devs misstake!!

She didnt died in my campaign, used my paragon save from me2 and in me3 i met her 3 times on citadel i think.

To save Miranda:

-Loyalty in ME2 as far as I know.
-Read the E-Mail concerning Kai Leng and when meeting with her per video conference, tell her about Kai Leng. 
-When she invites you to visit her personally you have to give her the Alliance resources ("paragon"-option on the RIGHT side of thewheel. Not coloured).
-No romance is needed

Then she will be alive after the cutscene at the end of the Horizon mission. I dont think it matters how you chose to solve the problem with her father. I used the intimidate option and then Miranda killed him and lived.

I confirm this as NOT being necessary for her survival. Just make sure you have her loyalty and you talk to her on the Citadel all three times. Not sure about resources though, probably they're a prerequisite(I've granted them anyhow, since I'm Paragon mostly).

It's simple: if you romanced her in 2, you have to keep it up if you don't want her to die. If you break it off, she dies regardless of everything else.

I played it for the 3th time (Restart old save) first i ran to the tower and she survived but died when i explored the thwe way to the elevator. I guess there is a certain time limit from the moment you see Kai Leng attack her till you get in the room to start cut scene?

She died my first play through, lived 2nd and died my third. The only thing I may have done differently is not get the 3rd meeting at the citadel 3rd time through. Wondering if it was because I got a cab somewhere else rather than the docking area at the citadel?
