These troubles begin on my 2nd playthrough:

Gunning down Dr Eva - after she is downed, the game freezes

Extreme mid-freezes - during the rescue of the female krogan

Citidel missing objectives: some missions refused to let me complete them; Aria's Eclipse is one of them, mission remains incomplete and won't let me talk to Seyn even though he is sitting there!


At times I suspect the Ashes DLC could be involved.
