Just finished my second go...
Final squad was Kaidan and Garrus
Kaidan was LI
Destroy ending (6600 military strength, with Shepard breathing ending)
Kaidan and Javik step out

The main thing I did differently here, was that I actually TALKED to Javik. I had him in my squad ONCE for an N7 mission. All the other listed choices are exactly the same as my first playthrough, where I had Kaidan and Garrus step out (Control 4500 total military strength).
Consensus: if you want your LI and BFF to step out, don't talk to Javik :/

I did the Destroy ending on my latest playthrough with EDI and Kaidan (who is my LI). I saw some people in other threads saying they had EDI walking out if she was in the party with your LI (despite the Destroy ending), so I wanted to try it.

I got Kaidan and Vega though. :/

-- I chose Destruction

-- I have the Javik character in DLC but he didn't go on many missions with my Soldier
-- Tali was my LI
-- Difficulty:  Insanity

Edit:  I almost never took Tali on missions with me.  However James [tank & inferno grenade] was in my squad most of the time.

Seriously?  

If I do Control with EDI and Ashley (LI) as my squad, then I get both of them walking out of the Normandy.
Synthesis is the same, but EDI is a pre-req there.

But with Destroy, EDI is replaced by Javik every time.

Destroy:

2a. LI -- if on the Normandy (excludes Miranda, Jack, Jacob; includes Traynor and Cortez)
2b. Random squad (as many have had people they barely talked to step out; Traynor and Cortez don't count for this)
3. Random squad

* EDI does not count as random in this ending -- obviously *

Control:
1. Joker
2a. LI -- if on the Normandy (excludes Miranda, Jack, Jacob; includes Traynor and Cortez) ... OR
2b. Random squad (see notes on Destroy ending)
3. Random squad

* EDI also counts as random squad in this ending *

That's a good question, but probably not.  If you turn around at all while running toward the beam in London, you'll see your party just standing there (or at least I did, and when I faced forward and looked back again they were gone). That's probably when they said "F-this" and went back to the Normandy.
